This Deposit Payment Agreement is established between the patient and SkinRenew. It is a legally binding agreement, and I consent to having my credit card charged.

This Deposit Payment Agreement pertains to a Consultation or Treatment at SkinRenew.
SKINRENEW APPOINTMENT - NO SHOW POLICY:
When we reserve time for our clients, we want you to feel valued. That’s why we strive to find a schedule that works for both parties.
We place great importance on your time and hope you will reciprocate. SkinRenew is consistently busy, so we highly recommend making prior appointments; however, walk-ins and last-minute appointments are welcome if time allows.
Please inquire about any cancellations, and we will do our best to accommodate your needs. Due to the medical nature of many treatments, appointments may occasionally exceed their scheduled time. We appreciate your patience and assure you of the same exceptional service and personal attention.
We send courtesy text reminders about your appointment, but these are just a courtesy. We kindly ask that you take responsibility for remembering your appointment date and time.
DEPOSIT:
SkinRenew has a deposit policy for scheduling.
A deposit is required when the appointment is booked. This deposit secures your reservation.
$75 - Deposit for Aesthetician
$150 - Consultation Deposit for Dr. Grace Kwon >> The consultation deposit can be applied to any services completed within one week after your consultation <<
$300 - Treatment Deposit for services over $500
CANCELLATION: SkinRenew requires FIVE (5) BUSINESS DAYS notice to cancel or reschedule if you wish to maintain your current deposit or receive a refund.
Once a deposit is made and your appointment is secured, the deposit is NON-REFUNDABLE if not canceled or rescheduled FIVE (5) BUSINESS DAYS before your appointment date. However, we understand that circumstances can change unexpectedly. If you are within FIVE (5) BUSINESS DAYS of your appointment, you may reschedule once using your original deposit.
If you are within THREE (3) BUSINESS DAYS of your appointment, your original deposit CANNOT be refunded. However, you can provide a new deposit to reschedule.
